Job Title:
Home Health Care Physical Therapist

Reports To: Director of Rehab
FLSA Status: Exempt

Essential Responsibilities and Duties: (Note: Other Duties May Be Assigned)

  • To provide physical therapy as prescribed by a physician which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities test.

  • To observe and record activities and findings in the clinical record

  • Report to the physician the patient’s reaction to treatment and any changes in the patient’s condition.

  • To instruct the patient and family in care and use of physical therapy devices.

  • To instruct other health team personnel including, when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient.

  • To instruct the family on the patient’s total physical therapy program.

  • Supervises the Physical Therapy Assistant in coordination with the Director of Rehab.

  • Participates in all required case conferences, utilization reviews, orientations, in-services and continuing education meetings.

  • Development and evaluation of the plan of care in partnership with the patient; representative (if any), and caregiver(s).

  • Providing services that are ordered by the physicians as indicated in the plan of care.

  • Preparing clinical notes.

  • Participation in the HHA’s QAPI program.

  • Compliance with the Agency’s Code of Conduct.

  • Patient, caregiver and family counseling.

  • Patient and caregiver education.

  • Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.


Qualifications:

  • Graduate of an Accredited School of Physical Therapy



Education and/or Experience:

  • One year of physical therapy experience is required.


Language Skills:

  • Must possess good communication skills, both written and verbal in English. Ability to communicate in other foreign languages is desirable.


Mathematical Skills:

  • Math skills required


Reasoning Ability:

  • Able to work under pressure and handle multi-task functions. Must be able to deal effectively with people. Must be self-motivated and resourceful.


Certificates, Licenses, Registrations:

  • Licensed as a physical therapist in the State of Florida.
