HOME HEALTH AIDE - PRN (LEE AND COLLIER COUNTIES)

Summary:
Provides personal care and assistance with ADL’s as required.
Essential Responsibilities and Duties: (Note: Other Duties May Be Assigned)
Personal care activities contained in a written assignment by a health professional employee of the home health agency which include assisting the patient with personal hygiene, ambulation, eating, dressing, shaving, and physical transfer
Other activities as instructed by a health professional employee of the home health agency for a specific patient and are restricted to the following:
Assisting with the change of a colostomy bag, reinforcement of dressing
Assisting with the use of devices for aid to daily living such as, a walker or a wheelchair
Assisting with prescribed range of motion exercises
Assisting with prescribed ice cap or collar
Doing simple urine tests for sugar, acetone or albumin
Measuring and preparing special diets
Measuring intake and output of fluids
Keeping records of personal health care activities
Observing appearance and gross behavioral changes in the patient and reporting to the registered nurse
Supervision of self-administered medication in the home limited to the following:
Obtaining the medication container from the storage area, if applicable
Preparing necessary items such as juice, water, cups or spoons to assist the patient in the self-administration of medication
Reminding the patient that it is time to take the medication prescribed
Observing the patient self-administering the medication.
Complying with infection prevention and control policies and procedures
Reporting changes in the patient’s condition
Honoring patient rights
Reporting any change to patient’s skin condition
Development and evaluation of the plan of care in partnership with the patient; representative (if any), and caregiver(s)
Providing services that are ordered by the physician as indicated in the plan of care
Preparing clinical notes
Participation in the HHA’s QAPI program
Compliance with the Agency’s Code of Conduct.
The home health aide shall not change sterile dressings, irrigate body cavities such as giving an enema, irrigate a colostomy or wound, perform a gastric irrigation or enteral feeding, catheterize a patient, administer medications, apply heat by any method, care for a tracheostomy tube, nor provide any personal health service which has not been included in the patient care plan.
Qualifications:
Successful completion of at least 75 hours of training, which shall include all the subjects required by AHCA or possess a Florida Certified Nursing Assistant (CNA) certificate.
Education and/or Experience:
Training must have been performed by or under the general supervision of a registered nurse
Language Skills:
Must possess good communication skills, both written and verbal
Mathematical Skills:
Math skills required
Reasoning Ability:
Able to work under pressure and handle multi-task functions. Must be able to deal effectively with people. Must be self-motivated and resourceful.
Certificates, Licenses, Registrations:
75 Hour Training Certificate from an accredited school or a CNA certificate.
